Every participant must sign and complete the following forms in resort. If under 18 forms must also be signed by a parent or guardian:

Liability Release and Assumption of Risk Agreement
Medical Statement and History
Standard Safe Diving Practices Statement of Understanding.

Participants of all ages undertaking the Bubblemaker or Discover Scuba Diving courses must sign and complete the Discover Scuba/Discover Scuba Diving Statement in resort before entering the water.
